2. Fill out as much of the form as you want.
3. When you are ready to stop, but have not finished entering all the data, just click on the "LOG OUT" button at the bottom of the page. This way all your information is saved for your next visit.
4. Return visits are initiated by going to "MY ACCOUNT", signing onto "ACCESS MY ACCOUNT" and entering the user name and password you originally established -- remember that both are case sensative -- then choose the desired action you want to perform. 
5. When you have done as much as you can, click "SUBMIT" on the last page of the application. Your file will be scambled and E-MAILED to our office, where it is downloaded to a program which unscrambles it for review.
